TEAM UP is thrilled to announce the launch of a Request for Applications (RFA), offering an exceptional funding opportunity for Massachusetts-based primary care practices serving children and adolescents to partner with the TEAM UP Center in advancing integrated pediatric behavioral health care. Apply for our newest cohort!
The TEAM UP Center invites Massachusetts-based primary care practices serving children and adolescents to apply to join the next cohort to implement the TEAM UP model. What is TEAM UP:Â Timely and Comprehensive Behavioral Healthcare for Children
TEAM UP equips pediatric practices to deliver comprehensive behavioral health services to patients from birth to young adulthood. The TEAM UP Modelâ„¢ is a team-based approach to integrating behavioral health into pediatric primary care. Developed with pediatric practices, the model includes a fully integrated multidisciplinary care team—Behavioral Health Clinicians (BHCs), Community Health Workers (CHWs), and Primary Care Providers (PCPs)—to promote healthy development and ensure early identification of behavioral health issues.Â
TEAM UP: A Proven Model of Care
Nearly a decade of evidence demonstrates that TEAM UP works. TEAM UP practices see first-hand the success of integrating mental and physical healthcare in a single, convenient setting. TEAM UP has been shown to have a positive impact on children and families, the behavioral health workforce, and the broader healthcare system. TEAM UP has trained over 400 professionals and engaged more than 15 practices to implement the TEAM UP model or build capacity for IBH care. Â
