The TEAM UP Center invites Massachusetts-based primary care practices serving children and adolescents to apply to join the next cohort to implement the TEAM UP model. The TEAM UP Center will provide practices with financial, technical, and operational support that includes in-depth clinical training, practice transformation support, and customized, data-driven technical assistance. 

What is TEAM UP: Timely and Comprehensive Behavioral Healthcare for Children
TEAM UP equips pediatric practices to deliver comprehensive behavioral health services to patients from birth to young adulthood. The TEAM UP Model™ is a team-based approach to integrating behavioral health into pediatric primary care. Developed with pediatric practices, the model includes a fully integrated multidisciplinary care team—Behavioral Health Clinicians (BHCs), Community Health Workers (CHWs), and Primary Care Providers (PCPs)—to promote healthy development and ensure early identification of behavioral health issues. 

TEAM UP: A Proven Model of Care
Nearly a decade of evidence demonstrates that TEAM UP works. TEAM UP practices see first-hand the success of integrating mental and physical healthcare in a single, convenient setting. TEAM UP has been shown to have a positive impact on children and families, the behavioral health workforce, and the broader healthcare system. TEAM UP has trained over 400 professionals and engaged more than 15 practices to implement the TEAM UP model or build capacity for IBH care.  

Eligibility Criteria  

  • Pediatric primary care or family medicine practice  
  • Located in Massachusetts  
  • Minimum patient panel of approximately 2,500 children and adolescents  
  • No planned changes to the EMR system within the next 36 months  
  • Commitment to participate in all practice transformation and evaluation activities  

Financial and Infrastructure Support 
Practices will receive scaled funding between $100,000 and $300,000 based on size, patient population needs, and infrastructure to support staffing, training, evaluation, and implementation activities. Practices will also receive a customized data dashboard powered by Relevant Healthcare, funded by the TEAM UP Center, to support data sharing, performance monitoring, and evaluation.
