TEAM UP By the Numbers Document from June 2017 to February 2022
We wanted to provide a comprehensive snapshot of all of the work that has been accomplished through the initiatives outlined in TEAM UP for Children Model. We would like to give special thank you to the 7 Federally Qualified Community Health Centers that have partnered with TEAM UP for Children: Brockton Neighborhood Health Center, Codman Square Health Center, The Dimock Center, DotHouse Health, Greater New Bedford Community Health Center, Lowell Community Health Center, and South Boston Community Health Center.
